When you submit a form for inquiry on services, please specify the provider you would like to be scheduled with and office (Fort Dodge, Humboldt, School-Based). Please additionally include the type of insurance you currently carry.
The contact form will be sent to the therapist of your choice if you specify. If you are requesting services through an Independent Contractor, please be mindful they monitor and track their own referrals and wait list including forms submitted through the practice website.
As a reminder, a submission is not a confirmation of scheduling. We do currently have a wait list.

The Heart of Play, LLC does not offer walk-in appointments or crisis counseling. Our team of therapists schedule with established clients by appointment only.
